WHY IT MATTERS
The biggest decision your family will make — usually made in a hallway, in a panic.
Home or facility. It shapes the rest of your parent’s life and costs tens of thousands of dollars. Most families decide it blind, under pressure, with no framework at all. This workbook gives you the same structure a nurse would use — so you decide with evidence instead of fear.
WHAT'S INSIDE
Five sections, one clear answer
SECTION ONE
Seven Pathways Self-Assessment
Rate each pathway from 1 to 5 — hydration, nutrition, sleep, mobility, pain, disease management, medications, and soul care. Score out of 40.
SECTION TWO
Detailed Pathway Assessment
The score tells you where you stand. This tells you why — a guided deep-dive into every pathway, with room to write.
SECTION THREE
The Real Cost, Side by Side
Fill in your real, local numbers. Home care versus facility, with 1- and 3-year totals. Most families are stunned by what they find.
SECTION FOUR
The Decision Framework
Bring the score and the numbers together, and answer the seven questions that lead to an honest decision.
SECTION FIVE
Your Next Steps
Turn the assessment into action: the weakest pathway to fix first, your 30-day moves, the team member you’re missing, and the conversation you need to have.

QUESTIONS
Honest answers
Is this a substitute for medical advice?
No. This workbook is a framework for thinking through your situation. If your loved one has complex medical conditions, multiple medications, or you’re unsure about any aspect of their care, please consult a healthcare professional.
How long does it take?
Set aside two to three uninterrupted hours. You’ll want their medication list and, if you have them, recent medical records or discharge summaries.
What if the score says facility?
Then that’s the truth you needed. If home is possible with support, home is almost always better for the person. But when it isn’t safe, a good facility is an act of love — not a failure. There’s no shame in either answer.
What if I want help working through it?
Book the Home vs. Facility Assessment and a nurse will do it with you in the home — with a real cost comparison and an honest recommendation. Or start with a free 15-minute call.
Get clear before you decide.
Download the workbook, set aside an afternoon, and answer the question honestly — before someone else answers it for you.
